Telehealth Informed Consent
By booking an appointment with Root + Revive Health Co., you agree to receive medical services via telehealth. Telehealth is the use of electronic communications to enable health care providers to evaluate, diagnose and treat patients remotely. During telehealth encounters you may be asked to share health information and images with your provider, and your provider may use live video, audio, and electronic systems to communicate with you.

- Nature of telehealth – Telehealth allows us to provide care without an in‑person visit, but it may not be appropriate for every medical condition. If your provider determines that an in‑person evaluation is necessary, you may be referred to a local provider or urgent care.
- Confidentiality and security – Our telehealth platform is encrypted and HIPAA‑compliant. Only authorized staff and healthcare providers will have access to your information.
- Limitations – Telehealth has limitations, including the inability to perform certain physical examinations. You have the right to refuse telehealth services or terminate a session at any time.
- Consent – By using our telehealth services you consent to receive health care via telecommunication technology and acknowledge that you have read and understand these terms.
HIPAA Privacy Policy
Root + Revive Health Co. follows all requirements of the Health Insurance Portability and Accountability Act (HIPAA). We are committed to protecting your personal health information (PHI) and maintaining confidentiality.
How we protect your information
- We collect only the health information necessary to provide care and operate our practice.
- Your PHI is stored in a secure, HIPAA‑compliant electronic medical record system with access restricted to authorized personnel.
- We do not share your information with third parties unless required for your care (e.g., pharmacies or laboratories) or by law. When disclosure is necessary, we provide only the minimum information needed.
- You may request access to or copies of your medical record at any time. We retain records for at least seven years in accordance with federal and state regulations.
Security Policy
The security of your data is important to us. We use industry‑standard safeguards, including encryption, secure servers and multi‑factor authentication, to protect against unauthorized access, use or disclosure of your personal information. Our staff follow strict internal procedures for handling PHI and undergo regular training on data security.
Financial Policy
Root + Revive Health Co. operates as a cash‑pay practice. We are not credentialed with insurance plans and therefore do not submit claims to insurers.
Payment terms
- Payment at the time of service – All services (including consultations, follow‑ups and prescription management) must be paid in full before or at the time of your appointment. You will be asked to place a valid credit or debit card on file when booking your first visit.
- Refunds and cancellations – Appointments cancelled with at least 24 hours’ notice will be refunded in full. Cancellations with less than 24 hours’ notice or no‑shows may be subject to a cancellation fee of $50.
- Billing for medications – Medications and compounded prescriptions may incur additional charges. We will inform you of these costs before proceeding. Payments for medications dispensed through partner pharmacies are processed separately.
- Root + Revive Health Co. uses Stripe to securely process all payments.
By completing a purchase on our website, you authorize Root + Revive Health Co. to charge your selected payment method for the service or membership you have chosen.
Initial Consult Fee
- The initial consult ($175) is a one-time, non-refundable fee that covers your first telehealth visit and development of your personalized care plan.
Lab testing and medications are billed separately.
Membership Billing
- When enrolling in a monthly membership, you agree to recurring monthly charges of $75.
This fee includes follow-up visits, unlimited portal messaging, unlimited lab reviews, and discounted sick visits.
You may cancel your membership at any time. Your plan will remain active through the end of the billing period already paid.
Lab Charges
- Lab fees (including the optional 60+ biomarker panel) are paid separately and are not included in membership dues.
Lab pricing is transparent and will be shown prior to ordering.
Refunds
- Payments for completed consults, lab panels, or months of membership that have already begun are non-refundable.

Prescription & Treatment Policy
Root + Revive Health Co. provides medical weight management (GLP‑1 agonists), hormone replacement therapy (HRT) and general wellness care for adults. Prescriptions are issued only after a thorough review of your medical history, laboratory work and a telehealth visit with our provider.
Key points
- Appropriate use – Medications such as GLP‑1 receptor agonists and hormones are prescribed when clinically appropriate and may not be suitable for every patient. High‑dose regimens or athletic‑performance use is not permitted.
- Monitoring – We require periodic follow‑up visits and laboratory testing to monitor therapy effectiveness and safety. Failure to comply with recommended follow‑ups may result in discontinuation of therapy.
- Pharmacies – We partner with reputable compounding pharmacies to dispense medications. Once a prescription is issued, you are responsible for payment to the pharmacy.
- Refills – Refill requests require an active patient‑provider relationship and may necessitate a follow‑up visit. We do not provide emergency refills or prescriptions beyond what is medically appropriate.
Cancellation & No‑Show Policy
To respect our provider’s time and allow access for other patients, we require 24‑hour notice for appointment cancellations. If you cancel with less than 24 hours’ notice or fail to attend your scheduled appointment:
- The $50 fee may be charged to the card on file.
- Repeated no‑shows may result in dismissal from the practice.
We understand emergencies happen. Please contact us as soon as possible if you are unable to attend your appointment.
Record Retention Policy
We maintain all patient records in compliance with federal and state laws. Medical records are stored securely for at least seven years after the last date of service. You may request copies of your records by contacting us in writing. A reasonable fee may apply for administrative costs.
